NOT include any complete, personally identifying information, such as
photograph,
address, social security number, or telephone number. I understand that
4Safedrivers.com
may use an online identity verification system that may require me to prove
who
I am. This may include a multiple-choice quiz on information about
me. As a part of that process, 4Safedrivers.com records my internet protocol
address for
my computer. I understand that 4Safedrivers.com will not refund the fee
I am paying if I
refuse to verify my identity if required. I understand that I may be
required
to follow 4Safedrivers.com's instructions on how to prove who I
am by providing
copies of records. I understand that if asked to verify my identity and
I am unable
to do so (either through the electronic tool or through copies of
